第一篇：中秋節請假條

敬愛的老師：

月是故鄉的明。每逢佳節倍思親，渴望闔家團圓的温暖，老師，我已經找到了我要請假的理由，那就是：與家人一起共度良宵。請許可吧，老師，中秋節快樂。

今晚是一個月圓之夜。已往的今天，學校組織我們拾棉花，已經連續三年沒有和奶奶爺爺共度中秋節了。如果老師給我們准假，我們會真誠的對您説五個字：中秋節快樂。如果非要加一個期限，我們希望是年年如此。準吧，老師，不要猶豫了。謝謝老師。

第二篇：請假條的格式

請假條的格式

由於某種原因你不能去上學，或不能參加某項活動，就應向老師請假。

怎樣寫請假條呢？

請假條一般應包括以下幾個部分：

（1）稱呼。要項格寫，並在後面加冒號。

（2）正文。要另起一行，空兩格。主要寫明請假的原因和請多長時間假。

（3）請假人姓名和日期。寫在正文的左下方。

李老師：

我因要去參加少年宮的大合唱彩排，今天下午的班隊活動不能參加，特此請假。

宋教師：
